What is the simplest way to automatically populate a form? [on hold]

I am updating a site for a training company. Each course has its own page, on which the dates of upcoming courses are listed. At present, the customer manually fills in a booking form with all the relevant information, including the course name, venue, and dates.

What I would like to implement is a system whereby, when the user clicks the relevant date, the booking form (which is on a separate page) is automatically populated with the course name and date. Is that possible? Would it be simpler to have a form on each page and then have just the date and venue automatically filled in on click?
